By Colorado law, homeowner associations can publish Member Directories provided certain requirements are met.

In order to be included in the Canyon View Home Owners Association Member Directory each Association Member (homeowner), or resident, must authorize the Association – in writing – to include their name, address, and telephone number.  Members and  / or residents also have the option of allowing their email addresses to be published in the directory.  If you want to be included in the Member Directory, please complete and submit this form. 

If you have previously provided an authorization and now want to revoke that authorization, a Revocation option is provided at the bottom of this form.

Note: The Association and / or Association Members are not allowed to share the published list for any commercial, non-HOA related purpose.  The list is maintained at the Association office and is available for distribution to all members and  / or residents upon request.
